Functional Safety Management Systems
FSC are able to undertake third party audits on procedures and techniques for compliance to recognised safety standards including IEC61508 and IEC61511.
HAZOP
HAZOPs are largely
carried out in the client’s offices with specialist support provided
by FSC.
HAZAN
FSC has access to a large database of reliability data for all types of components and subsystems. Using this data, the output of the HAZOP studies can be quantitatively analysed, typically using Layer of Protection Analysis (LOPA), Fault Tree Analysis and Reliability Block Diagram techniques. Each method can be used in assignment of SIL requirements (SIL targeting) and can be calibrated to the client’s specific risk criteria. The ALARP (As low as Reasonably Practicable) principle are applied where appropriate.
Safety System Reliability Studies
Assessments of proposed or existing safety systems can be analysed using data from client, vendors or from FSC’s own database. The analyses can be used to determine hardware reliability, in terms of probability of failure on demand / failure rate, and architectural constraints, as part of the SIL assessment process.
General Reliability / Availability Studies
The analysis of the equipment under control can be extended to cover potential environmental impact and loss of production. Again FSC has access to extensive reliability data.
Machinery Risk Assessment
FSC utilise a software-based semi-quantitative method for hazard identification, risk estimation and risk evaluation as part of achieving compliance with the Provision and Use of Work Equipment Regulations 1998 (PUWER).
Dispersion Modelling / Consequence Analysis
For consequence analyses FSC uses the DNV Software PHAST (Process Hazard Analysis Software Tool). PHAST examines the progress of a potential incident from the initial release to far-field dispersion of flammable and toxic materials for both gaseous and two-phase releases.
Reliability Centered Maintenance
Reliability Centered Maintenance (RCM) can provide a cost-effective maintenance strategy by creating a rationalised schedule of maintenance tasks which address the dominant causes of maintenance-preventable failures, as well as optimising spares holding.

Kenneth Simpson has been associated with safety related systems design and also with their assessment for 25 years. He is a member of the IEC61508 and IEC61511 drafting committees and also of the IGasE(SR15) panel, which writes the gas industry guidance. Following a career in aerospace, Ken has spent 25 years in the control and safety system industry and has written a number of papers on the topic and gives frequent lectures.

Dr David Smith has been directly concerned with reliability, safety and software quality for over 30 years and has written a number of books on the subject as well as numerous papers. His PhD thesis was on the subject of reliability prediction accuracy and common cause failure. He chairs the IGasE panel, which develops its guidelines on safety related systems (now in its third edition) and also made contributions to the development of IEC61508.
